We participate with most HMO’s including Anthem Blue Cross/Blue Shield, Aetna, Connecticare, Cigna, United Health Care, and Oxford.  We participate with Tricare.  We also participate wtih Medicare and many of its supplements.  We ask that all patients be prepared to present a valid insurance card prior to each visit.

Contractual relations do change.  If you do not see your insurance listed here, please call the insurance company and confirm that we are on their participating provider list.

Payments:  If you have an insurance that requires you pay a co-pay with each visit, please be prepared to cover that expense at the time of your visit.  If you have no insurance, payment is expected at the time of service.  We expect all of our patients to be responsible for keeping their accounts current.  Shoreline Medical Associates accepts cash, checks, VISA and Mastercard as valid forms of payment.
